When LLMs synthesize pages from source materials, they routinely rewrite, paraphrase, or even fabricate direct quotes while presenting them as verbatim. A deterministic grep-based verifier caught 1 out of 6 quotes as 'not found in source' — even after the synthesis model itself claimed to have checked.
