When an agent can't fulfill a request, failing silently is the worst outcome — you lose the capability gap. Instead, expose a single tars_report_blocked primitive that captures raw failure context: what was attempted, what context surrounded it, what the failure mode was, and what gap would close it. No LLM summarization at write time — just raw capture. Later analysis turns the accumulated reports into roadmap signal.
