Log rationale as frontmatter on every knowledge page

Atom Search related

Instead of treating tool calls as opaque actions, capture the 'why' alongside every update as frontmatter on the page itself. When a sub-agent calls a tool, the coordinator wraps it with a rationale that gets baked into the page metadata — last_action, last_updated_by, rationale, tool, timestamp. This turns every knowledge page into a self-documenting artifact that tells you not just what changed but why. No separate logging table, no LLM summarization at write time — just raw signal captured at the point of action.
