DB-only fixes get silently undone by reconciling systems

In any system with a reconciliation loop, fixing only the canonical store (the DB) is a trap — the next sync cycle reads from the source of truth it trusts and overwrites your edit. The fix has to land everywhere the reconciliation reads from, or it gets wiped. This is the silent-revert class of bugs: everything looks correct for an hour, then it isn't.
